Required Annual Permissions, Notices, and Opt Out Forms Available

4J's required annual permissions, notices, and opt out forms are ready for families.

Parents/guardians must complete these forms each school year to renew permissions or opt-outs. You can download the forms from the 4J website at the links below, or come to the Churchill Front Office for a paper copy.

For high-school students, parents only complete the Walking Field Trip portion.

Includes:

Objection to Release of Directory Information (includes options for school directory, yearbook, and publicity)
Notice of Human Sexuality Instruction (Complete only if you wish to excuse your student from human sexuality instruction)
Disclosure of Information to Military or College Recruiters (This information is shared with military recruiters early in the school year. Complete this form now to opt out of military recruitment. Must be renewed annually.)

Ice Cream Social Tomorrow

Our Back-to-School Ice Cream Social is tomorrow evening, 4:00 – 6:00 pm. Rain or shine, we'll have sweet treats for all to enjoy.

This is not a registration event. However, office staff will be there to answer questions and help with some back-to-school tasks.

While you're there:

Buy a Churchill hoodie or t-shirt. Cash or check only.
Meet the Churchill administrators.
Meet teachers from 4J's Center for Applied Learning and Community Impact (CALCI).
